The following qualifies as chronically sick or disabled:

 

  • You have a physical or mental impairment which has a long-term and substantial adverse effect upon your ability to carry out everyday activities.
  • You have a condition that the medical profession treats as a chronic sickness, such as diabetes or Alzheimer’s disease. It does not include an elderly person who is not disabled or chronically sick or any person who is only temporarily disabled or incapacitated, such as with a broken limb. If you are unsure, you should seek guidance from your GP or other medical professional.
  • You have a terminal illness

Step 2 - Fill in this Form

 

Please use the email address you registered in Step 1

 
 

Step 3 - Wait for Confirmation

 

Smart Life Solutions will check the information provided on the form above and, if acceptable, your account will be marked as VAT exempt. You will be notified by email within 8 working hours if your application has been successful. 

 

Step 4 - Place Your Order

 

Finally, once you have received email confirmation from us, select the product you would like to purchase and proceed to Checkout to place your order. VAT won’t be charged.

 

Please note that at Checkout you must use the same email that was used in Step 1 and Step 2. Otherwise, you will be charged VAT.
